1. Download, print and complete
the Forum Publications Order
Form.
2. Make all checks or money orders
payable the Donors Forum. All orders must include payment
for shipping and handling.
3. Send payment, including shipping,

If purchasing by credit card (Visa,
MasterCard, American Express), you may also fax in your
order form.

2. Please include your credit card
number, expiration date and signature on the form where
noted. All orders must include payment for shipping
and handling.

Publications
Policies: Shipping, Discounts and Refunds
All orders must be paid in full,
including shipping and handling charges as applicable,
before they can be processed. Shipping charges are calculated
as part of all online orders. Publications will be shipped
within 10 business days of receipt of order and appropriate
payment.
Only current Donors Forum Members,
Associate Members and Partners are eligible for discounts
as noted. Bulk discounts are available on many publications.

The Donors Forum does not offer refunds on publications,
except in the case of product defect or Donors Forum
